Your business name, ABN and address, the principal contractor, the contract number, the period of work, the date of your payment claim, and your certificate of currency.
We put your details into the current official form and check the dates are continuous with your last statement and that the certificate of currency is attached.
You read the declarations about workers compensation, wages and payroll tax, tick the ones that are true, then print and sign it. We never tick or sign for you.
This form comes round with every claim, and mistakes usually come from rushed dates or a missing certificate. We do the typing and the date checks. The declarations stay with you, because they're legal statements only you can make.

No. We fill in a public form from the details you give us. You decide whether each declaration is true. If you're unsure about payroll tax or workers compensation, ask your accountant, Revenue NSW or your insurer.
The Revenue NSW form says it is prepared for section 175B of the Workers Compensation Act 1987, Schedule 2 Part 5 of the Payroll Tax Act 2007 and section 127 of the Industrial Relations Act 1996. If it's completed in line with those provisions, the principal contractor is relieved of liability for workers compensation premiums, payroll tax and remuneration payable by the subcontractor.
No. The declarations are yours. The form notes that giving a statement you know to be false is an offence, so only you can confirm them.
The current Revenue NSW OPT 011 form. The version we checked is dated June 2026. If Revenue NSW updates it, we switch to the new version.
The form says the principal contractor must keep a copy for the periods set in the relevant laws, currently up to seven years.
We use them only to deliver your order. We delete them 30 days after delivery and never use them to train AI models.
